Physical therapists duly registered in the Philippines may avail of PPTA Lifetime Membership.

STEP 1

Applicant pays the registration fee Php 2,000. If the applicant is living abroad and wish to send the documents on the preferred address, international courier fee will apply depending on the site of destination.

a. For local applicants: deposit fee through Security Bank

b. For applicants based abroad: use the following swift code: SETC PHMM

Note: Scanned copy of validated deposit slip must be sent to PPTA as proof of payment within a week only. 

STEP 2

Applicant fills out PPTA membership electronic application form found at the bottom of membership application process from www.philpta.org

STEP 3

Applicant uploads all the requirements:

a. Accomplished electronic membership application form (marked “original” for new members, or “update” for other purposes)
b. Scanned copy of updated PRC license
c. Scanned copy of validated deposit slip (kindly write applicant name and signature clearly)

STEP 4

Upon receipt of all required documents, Membership Committee reviews application. Applicant’s registration status is verified against Professional Regulation Commission list of registered physical therapists.

STEP 5

Application is approved and acknowledgment is sent back to applicant via e-mail within one (1) month of receipt of all documents. Response shall include PPTA membership number. New PPTA lifetime members may start enjoying their privileges upon receipt of PPTA membership number through e-mail.

STEP 6

Membership Committee forwards PPTA membership ID and certificate via courier.

PPTA will not be held responsible for documents not delivered by the courier because of an invalid address. A new set of documents may be issued, but in such case PPTA reserves the right to charge the applicant anew.

Only the PPTA Membership Committee is authorized to process membership applications and queries concerning membership through philptamembership@gmail.com.
